    Understanding Rescue Breathing

    Rescue breathing is an emergency procedure used to provide artificial ventilation to someone who is not breathing or is breathing inadequately on their own. It is a fundamental component of basic life support. The technique involves a rescuer blowing air from their own lungs into the victim’s mouth, forcing air into the victim’s lungs. This process mimics natural inhalation, delivering essential oxygen to the bloodstream when the person’s own respiratory system has failed. The goal is to keep the brain and other vital organs oxygenated until the victim can breathe on their own or until advanced medical help arrives.

    This intervention is especially critical in drowning incidents. When a person drowns, water fills the lungs, causing respiratory arrest. The heart may continue to beat for a short time, but without oxygen, it will soon stop, leading to cardiac arrest. By performing rescue breathing immediately after removing the victim from the water, you can often restore oxygenation and prevent the situation from deteriorating into full cardiac arrest. It is also a key skill used during cardiopulmonary resuscitation (CPR) for victims of cardiac arrest and respiratory failure from other causes, such as choking or drug overdose.

    Preparing for Rescue Breathing

    To deliver rescue breaths effectively, a few crucial preparatory steps are necessary. Taking the time to assess and position the victim correctly ensures that the breaths you provide will be effective.

    Assess the Victim’s Condition

    Evaluating the victim’s status is the most critical first step in providing emergency care. This quick assessment determines whether rescue breathing is necessary.

    • Check Responsiveness: Once you have removed the victim from the water and are in a safe location, gently tap their shoulder and shout, “Are you okay?” to see if they respond. If there is no response, the person is unconscious and needs immediate help.
    • Open the Airway: If the victim is unresponsive, you must open their airway. Use the head tilt-chin lift maneuver. Place one hand on their forehead and gently tilt their head back. Use the fingers of your other hand to lift the chin. This action pulls the tongue away from the back of the throat, clearing the path for air.
    • Check for Breathing and Pulse: With the airway open, look, listen, and feel for normal breathing for up to 10 seconds. Watch the chest for any movement, listen for breath sounds, and feel for air on your cheek. At the same time, you can check for a pulse at the carotid artery in the neck. If the person is not breathing but has a pulse, begin rescue breathing immediately. If there is no breathing and no pulse, start full CPR.

    Position the Victim

    Proper positioning is critical for successful rescue breathing. An incorrect position can block the airway, preventing air from reaching the lungs.

    • Lay the victim flat on their back on a firm, level surface. This provides stability and makes it easier to access their airway and perform chest compressions if needed.
    • Kneel beside the victim’s head and shoulders. This position allows you to easily perform the head tilt-chin lift or jaw thrust and deliver breaths without having to move around.
    • Re-apply the head-tilt-chin-lift maneuver to ensure the airway remains open. If you suspect a head, neck, or spinal injury (for example, if the person was diving into shallow water), use the jaw-thrust maneuver instead. This involves kneeling above the victim’s head and using your fingers to grasp the angles of their lower jaw and lift it forward, without tilting the head.

    Ensure a Clear Airway

    Before delivering breaths, it is vital to make sure nothing is physically blocking the airway.

    • After opening the airway, quickly look inside the victim’s mouth for any visible obstructions, such as mud, seaweed, or vomit.
    • If you see a blockage, turn the victim’s head to the side (if no spinal injury is suspected) and clear the material out with your fingers. A finger sweep should only be performed if you can see the object. A blind finger sweep can push an object deeper into the throat.
    • Remember that in drowning cases, the primary “obstruction” is water in the lungs, which you cannot remove. Do not waste time trying to force water out. The priority is to get oxygen in.

    Check for Proper Technique

    Your hand positioning and technique are what make rescue breathing effective.

    • When performing the head tilt-chin lift, ensure you are lifting from the bony part of the chin, not the soft tissue under the neck, which could inadvertently block the airway.
    • Prepare to create a tight seal over the victim’s mouth with your own. This prevents air from escaping and ensures the full force of your breath enters their lungs.
    • Mentally review the rhythm of the breaths you will provide: one breath every 5-6 seconds for an adult, and one breath every 3-5 seconds for a child or infant.

    Techniques for Delivering Rescue Breaths

    Once you have prepared the victim, you can begin delivering breaths. The technique must be precise to be effective.

    Position the Mouth

    Creating an airtight seal is essential to ensure the air you provide reaches the lungs and doesn’t leak out.

    • Maintain the head-tilt-chin-lift to keep the airway open.
    • Use the thumb and index finger of the hand that is on the victim’s forehead to pinch their nose shut. This prevents the air you blow in from escaping through the nostrils.
    • Take a normal breath (not a deep one) and cover the victim’s mouth completely with your own, creating a tight seal. For an infant, you will cover both their nose and mouth with your mouth.

    Deliver the Breath

    Each breath should be delivered smoothly and effectively.

    • Blow steadily into the victim’s mouth for about one second. The breath should be just strong enough to make the chest visibly rise. A normal breath from the rescuer is sufficient; blowing too hard can force air into the stomach, causing vomiting.
    • As you deliver the breath, watch the victim’s chest out of the corner of your eye. A visible chest rise is the best indicator that your breath has successfully entered the lungs.
    • After the breath, break the seal, remove your mouth, and allow the victim’s chest to fall as the air passively escapes. Take another normal breath yourself before delivering the next rescue breath.

    Repeat as Needed

    Consistency is key to maintaining oxygenation until help arrives or the victim begins to breathe independently.

    • Continue providing one rescue breath every 5 to 6 seconds for an adult (about 10 to 12 breaths per minute).
    • For a child or infant, deliver one breath every 3 to 5 seconds (about 12 to 20 breaths per minute).
    • Periodically (every 2 minutes), recheck for a pulse to ensure the heart is still beating. If you can no longer find a pulse, you must begin full CPR, which combines chest compressions with rescue breaths. Continue rescue breathing until the victim starts breathing on their own, another trained rescuer takes over, or emergency medical personnel arrive.

    Two-Person Rescue Breathing Technique

    When a second rescuer is available, performing rescue breathing as a team is more effective and less tiring. Professional responders commonly use this technique, but lay rescuers can adopt it if both are trained.

    Positioning the Victim

    The division of labor in a two-person rescue makes the process more efficient.

    • Rescuer 1 positions themselves at the top of the victim’s head. Their primary job is to maintain an open airway using the head-tilt-chin-lift or jaw-thrust maneuver and to hold the mask (if available) in place.
    • Rescuer 2 positions themselves at the side of the victim, ready to deliver breaths. This rescuer’s focus is solely on the timing and quality of the ventilations.

    Creating a Seal

    The two-person technique provides a superior seal, which is its most significant advantage.

    • Rescuer 1 uses both hands to secure the mask or to maintain the airway for mouth-to-mouth resuscitation. If using a mask, they can apply the “C-E grip” with both hands, forming a “C” with their thumbs and index fingers over the mask and an “E” with their other fingers to lift the jaw. This two-handed grip creates a much more reliable seal than a one-person rescue allows.
    • With the seal secured, Rescuer 2 can focus entirely on delivering smooth, effective breaths.

    Advantages of the Two-Person Technique

    Teamwork provides several key benefits in an emergency:

    • It ensures a better and more consistent seal, reducing air leakage and making each breath more effective.
    • It is less fatiguing for rescuers, allowing them to perform high-quality resuscitation for a more extended period.
    • One rescuer can monitor the chest for rise and fall while the other delivers the breath, providing immediate feedback on the effectiveness of the ventilation.
